SAGEvirtual Service Provider Qualifications

To be eligible to receive SAGEvirtual client referrals, clinicians must provide proof of the following:

• Liability insurance certificate to a minimum of $2 Million (to be done annually)
• A copy of their resume
• A copy of their highest attained degree (Masters level minimum requirement)
• A copy of registration with the College or Association of the clinician’s jurisdiction
• A Criminal record check and vulnerable persons check
